Assessment website privacy notice
Updated 1 October 2026. This notice covers the enquiry form and Practice Health Check on this website, operated by Paul Hannigan at Dove Digital.
What you share
The enquiry form collects your name, email address, clinic name, preferred contact method, optional phone number, selected topic, and any website address or message you choose to provide. The Health Check calculates scores in your browser. When you submit your contact details to view the full results, it sends your name, email address, optional clinic name and phone number, preferred contact method, overall score and a summary of the six assessed areas.
Please provide business contact details only. Do not include patient names, medical records or other patient information.
How your details are used
Your submission is sent to Paul at assessment@dovedigital.agency so he can respond to your enquiry or follow up about your practice’s assessment. Sending either form does not subscribe you to a newsletter. Health Check results appear on screen; this website does not automatically email them to you.
Services involved
Cloudflare hosts the website. Web3Forms processes form submissions and delivers notification emails to Dove Digital’s inbox. Copies may be held in Web3Forms and the email service used for that inbox.
Web3Forms uses infrastructure and spam-prevention providers and may process data internationally. Its privacy policy describes submission storage, retention and those providers. See the Web3Forms privacy policy.
The assessment and Health Check pages also load Cloudflare Web Analytics to measure website usage and performance, and Google Fonts to display their typefaces. These services receive browser requests. See Cloudflare’s privacy policy and Google’s privacy policy for their handling of data.
Questions or requests about your information
For questions about storage or retention, to request access, correction or deletion of your submission, or to raise a privacy concern, contact Paul at assessment@dovedigital.agency. Include enough information to identify the enquiry, but do not send passwords or patient information.
